Mosaic Wellness is committed to helping clients understand how to access their health records and where to find information regarding professional licensing and consumer complaints.

Requesting Your Health Records

If you would like to request access to or a copy of your health records, please complete the Mosaic Wellness Client Request for Health Records form.

To request your records:

1. Download and complete the Client Request for Health Records form.

2. Identify the Mosaic Wellness provider who provided your services.

3. Submit the completed form directly to your treating provider.

4. Your treating provider will review and respond to your request and may contact you if additional information or identity verification is needed.

If you are unsure how to contact your treating provider, please contact Mosaic Wellness for assistance routing your request to the appropriate provider.

Mosaic Wellness does not centrally process or release clinical records on behalf of its independent providers. Requests for clinical records are processed by the provider who rendered the services.

Professional Licensing & Complaints

Behavioral health professionals practicing at Mosaic Wellness are independently licensed or credentialed according to the requirements applicable to their profession.

The Texas Behavioral Health Executive Council (BHEC) oversees the licensing and regulation of professional counselors, marriage and family therapists, social workers, and psychologists in Texas.

Consumer & Health Information Privacy Complaints

Texas law provides consumers with rights regarding the privacy of and access to their health information.

If you believe your protected health information has been improperly used or disclosed, or you wish to file a complaint concerning your rights under the Texas Medical Records Privacy Act, information and complaint options are available through the Office of the Texas Attorney General, Consumer Protection Division.
